Scantron Compatible Forms for FDC Scanner

IMPORTANT INFORMATION:

The Scantron Scanner at the Faculty Development Center supports the forms listed below.
Please make sure that you use one of the forms listed below in order to use the FDC Scanner. For the first test of each semester, Form F-1712-PAR-L is highly recommended. You may wish to continue to use it throughout the semester. Please read the notes for drawbacks of other forms.
For your first exam or quiz you can auto-enroll your students into your ParScore class by using one of the forms that are marked "Auto-Enroll".
If you are a full-time or part-time faculty member at Cal State Fullerton, you may request a ParScore account by emailing sgaitan@fullerton.edu

Pink, 8.5” x 11” (Auto-Enroll)
Form Information:	The F-1712-Par-L form (above) is highly recommended. It is the form of choice for the first exam of the semester as it will enable the auto-enrollment of students into ParScore. Red, 4.25” x 11”
Form Information:	Drawback: No auto enrollment. Avoid using for the first test of the semester. Useful once students have been auto-enrolled in ParScore.	Price: $0.10 each Red, 4.25” x 11” Side 1: 100 questions (1-100) *Side 2:* 100 questions (101-200)
