Checklists to assist you

Download our checklist to track your progress towards earning your Teaching & Learning Academy Certificates. Check off the workshops you have participated in. You may wish to fill-in the date attended on the blank line following the workshop's title.

Checklists vary by program and year initiated - select the correct one for you!

To save the document to your computer, right-click the word PDF or DOC, and select Save Target As from the pop-up menu. The MS Word versions will enable you to select checkboxes by clicking on them (Use the forms toolbar to toggle between the checkbox and the ability to type words).

Please note that the same workshop may be offered in successive years. The title will be similar but may NOT be identical. You may only receive credit for taking the workshop once. If in doubt, contact the FDC.

Which Workshops Have You Taken?

Can't recall the workshops you've taken. You can check your FDC Workshop Transcript online by logging into the Workshop Registration site at: http://fdcweb.fullerton.edu/workshops/registration.htm

Once you have successfully logged in, click on Obtain a Transcript.

Core workshops will be identified with the category "TLAC" in the right-hand column on your transcript. Please note that ALL workshops you have enrolled in will display in your transcript, whether you attended or not. Workshops you have ATTENDED will be identified with an "A." In the sample transcript below, ONLY the workshops with the red box around them satisfy the core requirements. Other workshops attended (identified with an "A") may satisfy elective requirements. The remaining workshops were enrolled in but NOT attended. Occasionally workshops or events sponsored by other departments appear on a transcript. These workshops/events do not count towards fulfilling the TLAC requirements.

Certificate Requirements Complete? - Apply for Your Certificate!

Please inform us by e-mail when you have completed ALL the requirements for a Certificate.

  1. Type Application for Certificate in the Subject line.
  2. Also in the subject line, idenfity the certificate you are applying for. The choices are:
    • TLAC
    • TLAC - Community College
    • 4CTA
  3. Tell us what year you enrolled in the certificate program.
  4. Include your contact information.
  5. Send the e-mail to Susan Gaitan at sgaitan@fullerton.edu. View example below.

After verifying the certificate requirements have been completed, we will place you on our certificate list. Certificates are prepared and distributed at the end of the academic year.
